About the study

Help us make the move home from hospital easier.

This is a quality improvement study at the Jewish General Hospital. Quality improvement means trying a way of doing things, listening to the people who use it, and making it better.

By using POP and sharing your feedback, you can help the study team understand what information is clear, what is hard to find, and what patients and caregivers need after leaving hospital.

Who can participate

Patients and caregivers can take part.

The study is for people recovering at home after a hospital stay and for a family member, friend, or other trusted person helping them.

For patients

Use POP to keep your hospital instructions, medications, appointments, and next steps easier to find while you recover at home.

For caregivers

Help with the details they share.

Use your own POP account to help someone you care about keep track of the information they choose to share with you.

What to expect

Taking part, step by step

Taking part is your choice. Saying yes or no will not affect your medical care, your relationship with your care team, or the services you receive.

  1. Get a study code from the hospital study team or request one on this page.

  2. Use the code to create your POP account.

  3. Read the consent information in the app and decide whether you want to take part before answering anything.

  4. Complete a short questionnaire and use POP at home.

  5. Share what worked, what did not, and what would make POP more useful.

  6. The team may invite you to complete another questionnaire one month and three months after you leave hospital.

What happens to your information

What the study collects

If you take part, the study may collect:

  • Your answers to the questionnaires

  • Your feedback about POP

  • The date and time of your answers

  • Basic technical information, such as your browser and device time zone

  • A study code used to group your answers over time

Your study code connects your answers over time so the team can understand how your experience changes after you go home.

Who can see your information

Only authorized study team members can access the information collected for this study. Team members who review your answers can see your name with your responses.

People outside the study team cannot see your answers. Your name will not appear in reports or summaries produced from the study.

Some longer written answers may be shared in a private internal channel so the study team can review them quickly. If you choose to give a voice answer, the recording is stored with your other study answers; voice recordings are not automatically transcribed.

How your information is used and protected

The study team reviews your answers to understand your experience with POP and the move home. Your feedback may guide changes to POP’s wording, design, reminders, and explanations.

Your information is kept in a secure database. Access is limited to authorized study team members who need it for their work.

Information collected as part of the study will not be sold.

Taking part and stopping

Taking part is your choice. You can:

  • Ask questions before agreeing

  • Take part without using every feature of POP

  • Stop taking part at any time

You do not need to give a reason for stopping. Your decision will not affect your medical care, your relationship with your care team, or the services you receive.

If you stop, the team will not collect new answers from you. Information you already submitted will be handled according to the consent form you accepted in the app and applicable law. You can ask the study team whether it may still be kept or used and whether deletion or de-identification is available in your situation.

POP does not replace medical advice

POP helps you organize and understand your hospital instructions. It does not diagnose conditions, change your instructions, or monitor your health for your care team.

If anything in POP is different from your original hospital instructions, follow the original instructions and contact your care team. Contact them as well if you have a question or concern about your health.
